Payment Policy - due to the time and expertise required to preview your medical records to ensure a quality and comprehensive evaluation a $50 fee is due upon booking your appointment and is applied towards your total which is due at the time of service. If the appointment is canceled per policy the $50 will be refunded.

**Laboratory and other diagnostic tests are billed separately and not included in above fees.

1. What forms of payment are accepted?

Our preferred payment method is electronic payment via credit/debit cards via PayPal, CashApp, or Zelle.
We can also accept Swipe and Google Pay.

2. Do you take insurance?

Our focus is on your total wellness. Due to the nature of our work and our commitment to wellness, we are not limited or restrained by insurance policy makers or third parties that interrupt the patient-provider relationship. As such, we do not accept health insurance. But we will provide a receipt of services that may be presentable to insurance companies for out-of-network reimbursement to you. Additionally, we do not participate in Medicare or Medicaid programs.

4. Will you be my primary health care provider?

Our role is not that of a primary health care provider. We work with your primary care physician and specialist team to provide a holistic and integrative component in a collaborative fashion and provide areas of expertise that may extend beyond traditional medical practice, and utilize techniques that conventional physicians may not.

5. Is there anything I need to fill out before?

Prior to your appointment you will be asked to complete a medical questionnaire. It may be necessary to have all prior medical records sent to us as well so we can review all relevant information to maximize our ability to help you on your road to wellness.

6. What if I need to cancel my appointment?

We understand that life happens! We have a 24 hour cancellation rule for most appointments. If you need to cancel your appointment, please notify us at least 24 hours prior or you will be billed for the appointment. For extensive integrative/functional medical consultation appointments, we require 48 hour cancellation notice.
